Time Adjustment Form

Time Adjustments should be filled out for a variety of reasons:

​

1)If you forgot your key fob at home.

2)If you forgot to punch in/out. Please do not make this a habit.

3) If you loose your key fob completely. (You must request a new one from HR)

4) The time clock is not working.

5) You worked off-site at an event and were unable to use T-Sheets.
